Before we get started, please take some time to download and review my office policies so that you can effectively consent to treatment. If possible, print these and bring them with you to our first appointment. This will allow us to focus more on getting to know each other, and less on administrative tasks. If you haven’t already, make sure you’ve also logged into my online portal and registered as a new patient. This ensures that I have your contact information, insurance information, and basic medical history before we meet :)
